What the symptom is telling you

The VGT actuator: a separate failure, same symptoms
Every engine family in this class — Isuzu 4HK1, 6.7L Power Stroke, 6.6L Duramax, 6.7L Cummins, Fuso 4P10 — uses a variable-geometry turbo (VGT), which adds an electronically controlled vane actuator on top of the mechanical turbo itself. The actuator sticks or fails on its own, independent of the turbo's bearings and seals, and produces the same underboost/overboost codes. Before replacing the whole turbo, check whether the actuator moves freely through its full range — a stuck actuator with a healthy turbo is a cheaper fix.
Before you condemn the turbo: find the actual cause
A turbo rarely fails on its own — something usually starved it of oil or choked its airflow first, and that same cause will kill a replacement just as fast if it isn't fixed:
- Plugged or restricted oil feed/return line — starves the bearings
- Stretched oil-change intervals — accelerates bearing wear
- Plugged air filter — pulls the compressor wheel into a vacuum it wasn't designed for
- Boost leaks at clamps or cracked piping — mimic turbo underboost without any turbo damage at all
What a checked used turbo should include
- Shaft radial and axial play checked — excess play means the compressor wheel is touching the housing
- Compressor and turbine wheel condition inspected
- Oil seal condition checked
- VGT actuator movement verified through its full range, on variable-geometry units
Installation requirements
Change the oil and filter, clean or replace the intercooler and charge pipes, and prime the oil feed before first start — a dry bearing on startup is a fast way to kill a good turbo. On VGT units, clear codes and run the actuator relearn procedure if your engine's service manual calls for one.
